I'm a step grandma and their real grandma tried to call me nanny Emma but at 26 with 2 kids of my own under 5 it just felt wrong. I get called Emma, so we have Grandad and Emma on our cards. The poor kids has it confussing enough with his dads parents, his grandma and her partner who he calls gandad I think, his step grandparents and then us!!!!
Do what you feel is best, I know she has married him but that doesn't make him a grandad. Even more so as your step dad of 20 years i still in his life and your dad.

I don't see why not tbh. My DH parents are both remarried. His natural grandparents the kids call nan and grandad but the step-grandparents are called Nanny Sara and the other Grandad Roy. Why don't you just leave your child to decide what to call him. He might really get on with him and want to call him grandad or he might want to use his first name.
